At Gracewood Healthcare Ltd, we deliver high-quality Supported Living services across Northamptonshire, supporting young people transitioning into adult services, adults, and older adults who require flexible, person-centred care and housing to live independently and safely in the community.

Who We Support
Gracewood Healthcare provides personalised supported living for:
- Young people (17 years 11 months – 26 years) transitioning from children’s to adult services, including those leaving care.
- Adults (18–64) with mental health needs, learning disabilities, physical disabilities, or sensory impairments.
- Older adults (65+) who require ongoing support to remain independent, safe, and connected within their communities.
Each person’s support is tailored around their individual strengths, aspirations, and assessed needs.
Our Supported Living Models
We offer two structured levels of support, ensuring individuals receive the right care in the right environment:
Standard Supported Living (Shared Environments)
For individuals who are ready to live more independently but benefit from shared accommodation with others of similar age and interests. Encourages social inclusion and peer support. Ideal for people with stable needs who thrive in community settings. Support is flexible and planned around daily routines helping with cooking, budgeting, personal care, medication prompts, and accessing education or employment.
Enhanced Supported Living (Solo Placements)
For people with more complex, fluctuating, or higher-risk needs who require structured support, stability, and privacy. Delivered in self-contained accommodation with 24-hour staffing or intensive daily input. Designed for those with dual diagnoses, mental health challenges, or behavioural needs requiring consistent, therapeutic support. Provides a safe environment for recovery, reablement, and long-term stability. Our placement approach ensures individuals are matched carefully based on age, interests, compatibility, and needs, fostering harmony in shared homes and security in solo placements.

- Enhanced DBS-checked and reference-verified before starting work.
- Trained in Mental Health Awareness, Learning Disabilities, Autism, PBS, Safeguarding, and Person-Centred Care.
- Experienced in supporting individuals with diverse needs, including physical disabilities, sensory impairments, and complex mental health conditions.
- Supported through continuous supervision, reflective practice, and professional development to maintain competence and confidence.
